Use this letter to request a meeting with an employee who is off work on long term absence. Long term absence in this case is regarded as 4 weeks or more, however, the length of time can be changed to suit your own requirements.

I thought when a member of staff is off work and has submitted G.P. sick notes, the employer was unable to contact the employee as this may be regarded as harassment.
