No. Some jobs are physically taxing, or require a good level of health for health and safety reasons. This should be made clear at the outset. Completing this form will give a starting point for discussions regarding some health issues so the employer can make suitable adjustments if necessary. However, it should not be used to screen out less able potential employees from roles that could be adjusted; doing this could land you with a disability discrimination claim!
